Mercy Clinic chronic care/disease program
Complications from uncontrolled diabetes and persistent hypertension are among the leading causes of visits to the hospital's emergency department. To help reduce the number of hospital visits, the Mercy Clinic's Chronic Care Management Program focuses on providing consistent care to patients with chronic conditions such as Type 1 and Type 2 Diabetes Mellitus and Hypertension. Patients enrolling in this program are seen on a regular basis by specific providers to assist them achieve their health goals.
The physician can assess and develop a plan to improve or manage their chronic conditions based on the findings of blood sugar/blood pressure logs and hemoglobin A1c (HbA1c) blood tests.
